At 65 and older, apply through DCFS, or on NY State of Health if you don't need long-term care. Take the application and Supplement A to 60 Market Street in Poughkeepsie, 223 Main Street in Beacon or the Millbrook office. Bring proof of income and address, and your red, white and blue Medicare card.

The same form checks you for the Medicare Savings Program. It can pay the $202.90 a month Medicare costs you, usually taken out of your Social Security check. See Medicare and Medicaid together in New York.

If you need help at home soon, DCFS takes the forms for a fast start on an aide. That includes the Consumer Directed Personal Assistance Program (CDPAP), where you pick your own aide. See home care and CDPAP in New York.
